I enjoyed watching this video because it allowed me to consider how I could have this conversation in the future. This topic is difficult and is often squeezed in between other pre-therapy work up procedures and medical management. I will be the first to admit that I have done it poorly in the past, but am here to learn and to improve. I wish I could have video taped my own conversations so I could evaluate them further as well. To me, the conversation in general seemed a bit awkward especially at the beginning. There was so much space between the social worker and the patient that it seemed impersonal. Something else that really stood out to me was that the social worker briefly apologized that no one ever spoke to the patient regarding fertility effects of therapy, but the manner in which it was said seemed to kind of put down the primary oncology team. In difficult situations like this, I feel like it is important to review the facts and next steps without placing blame on any previous providers given the lack of knowledge of the acuity of illness or other factors at diagnosis. I felt like she normalized the experience of infertility by stating that 12% of the population has troubles with infertility regardless of cancer therapy, which may help the patient cope with the outcome. I also liked that she included his girlfriend in the conversation and offered to meet with them again together to answer further questions.
